I’m the Lead Physiotherapist working in our elderly medicine team, working across a number of wards which support patients with a variety of conditions. The nature of the job means we have to have an in-depth knowledge and understanding of how the patient is impacted by the condition they were admitted for, as well as any other conditions they may also have. I enjoy it because it allows me to use all of what I would describe as my ‘core’ physiotherapy skills, working with the respiratory, musculoskeletal and neurological systems, often with just one individual patient. It can be challenging at times to know which to focus on first, but I thoroughly enjoy rising to meet that challenge.
The purpose of my role is to ensure inpatients are able to be discharged in a safe and timely way once they are medically fit to go home. We work closely with the whole multidisciplinary team, especially Occupational Therapy, to assess our patients for risk and functional ability, and quickly identify the best discharge option for the patient. It may be for them to return home with additional support, inpatient rehabilitation or considering longer-term placements such as residential and nursing care. We also work with our community colleagues to offer support and rehabilitation to individual patients when they leave the hospital.
It’s not just about the patient. We liaise with families to understand how the patient was managing at home before they came into hospital, so we’re able to provide any of the additional support they patient and the family may need on discharge. We find that including patient and carers in this way helps us to ensure a timely discharge.
I enjoy the fast pace of working in elderly medicine. The patients may only be on the ward for a short period of time, so you have to quickly build a rapport, gain trust and work with the patient to make progress. So many of our patients have stories and life experiences to share, and it’s humbling to be able to share part of their journey. It’s hugely rewarding to support a patient to make sure they are safe and able to leave hospital to where they want and need to go.
